LPN Memory Care | AM Shift MondayFriday | 6:00 AM2:30 PM LPN Memory Care | PM Shift MondayFriday | 2:00 PM10:30 PM About the Position Willowick Assisted Living Moments is hiring two dependable, compassionate, clinically confident Licensed Practical Nurses (LPNs) to join our memory care team.

This position provides hands-on nursing care to residents while serving as a clinical leader and supervisor for the care staff.

The primary location is 304 Ogden Ave.

Clinton, WI Our LPNs play an important role in ensuring residents receive safe, consistent, person-centered care while maintaining communication with physicians, families, and the interdisciplinary team.

What You'll Do Administer medications safely and accurately according to physician orders and facility policiesProvide hands-on bedside care and support to residentsMonitor residents for changes in condition and promptly communicate concernsCommunicate significant changes in resident conditions to the appropriate clinical team members, physician, family, or responsible partyProcess and implement physician ordersMaintain and update Individual Service Plans (ISPs) as requiredComplete required nursing documentation and maintain accurate resident recordsMonitor resident needs and ensure appropriate follow-through by the care teamProvide direction, supervision, leadership, and support to caregiving staff during the shiftDelegate and oversee resident care responsibilities appropriatelyAssist care staff with questions, resident needs, and changes in conditionPromote a safe, respectful, and supportive environment for residentsWork collaboratively with leadership, families, physicians, pharmacies, and other healthcare professionalsFollow all applicable CBRF, DHS, medication administration, and facility policies and proceduresRespond appropriately to resident emergencies and changes in health statusServe as a clinical resource and leader for the care team What We're Looking For Current Wisconsin LPN license in good standingExperience in assisted living, memory care, long-term care, or a similar setting preferredStrong assessment and clinical judgment skillsAbility to recognize and communicate changes in resident conditionStrong medication administration skillsAbility to lead and supervise a team of caregiversExcellent communication and documentation skillsDependable, organized, and able to work independentlyCompassionate approach to working with older adults, particularly individuals living with dementiaAbility to remain calm and professional in challenging situationsCommitment to providing respectful, person-centered care Why Join Our Team?
