Med/Surg RN

JOB SUMMARY The Medical/Surgical & Swing-Bed Registered Nurse provides safe, compassionate, and coordinated nursing care to acute-care and Swing-Bed patients from admission through discharge.

The RN is accountable for assessment, care planning, intervention, evaluation, patient education, and communication with the interdisciplinary healthcare team.

Ages served range from pediatric to geriatric.

The RN manages patients with varied medical, surgical, post-operative, and rehabilitative needs and plays an important role in promoting recovery, mobility, independence, symptom control, and safe transitions of care.

The position requires strong organization, prioritization, communication, and the ability to manage multiple patient-care needs throughout the shift.

ESSENTIAL FUNCTIONS Plans, assesses, intervenes, coordinates, evaluates, and provides direct patient care using the nursing process and critical-thinking skills.Performs comprehensive assessments including physical, psychosocial, nutritional, functional, safety, spiritual, cultural, and educational needs.Recognizes changes in patient condition, establishes priorities, initiates appropriate nursing interventions, and communicates significant findings promptly to the provider and other members of the healthcare team.Develops, implements, and updates individualized plans of care in collaboration with patients, families, providers, therapy services, social services, pharmacy, and other disciplines.Provides care for medical, surgical, and post-operative patients, including pain management, wound and incision assessment, drains, tubes, IV therapy, mobility, respiratory needs, and prevention of complications.Provides Swing-Bed nursing care focused on rehabilitation, functional improvement, symptom management, participation in therapy, and progress toward discharge goals.Coordinates nursing care with PT, OT, and other rehabilitation services, including timely pain management and preparation of patients for scheduled therapy and treatments.Accurately reconciles, prepares, administers, scans, and documents medications in accordance with hospital policy and the rights of medication administration.Reviews laboratory and diagnostic results, recognizes significant findings, and takes appropriate nursing action, including timely provider notification.Completes accurate and timely documentation of assessments, patient status, interventions, treatments, medications, education, response to care, supplies, equipment, and other required charges.Coordinates admissions, transfers, and discharges and ensures complete handoff communication to receiving nurses, facilities, caregivers, or other providers.Provides patient and family education regarding diagnoses, medications, treatments, activity, diet, equipment, follow-up care, warning signs, and discharge instructions.Promotes patient mobility, independence, fall prevention, skin integrity, nutrition, elimination, comfort, and other activities that support recovery...
