Community Health Worker

COMMUNITY HEALTH WORKER (CHW) SUMMARY: The Community Health Worker is responsible for: the assessment of the social, emotional, and environmental needs of participants; the development of participant service plan, service arranging, follow up and monitoring, reassessment, and coordination of In-Home, transitional, and other supportive services.

The Community Health Worker role supports participants independence in the community which may include transition support, coordinating services, and on-going organization for those served.

REQUIRED DUTIES AND RESPONSIBILITIES: Attend in-home participant meetings and other community based appointments.Conduct comprehensive assessments for participants.Support participant through residential transitions by providing coordination and support services.Work with family members and other volunteer caregivers to maximize available services to the participant.Develop a plan of care identifying needed services, supports, and interventions in collaboration with team.Arrange and coordinate delivery of needed services (utilizing natural supports and community resources).Monitor participant Person Centered Service Plan to assure delivery of needed services.Evaluate and modify Person Centered Service Plan as required.Assist other care managers with questions resulting from reassessments and other situations.Participate in regular staff meeting and training events.Participate in community events to teach, assist, advocate, and forge community partnerships to manage needs not addressed by natural supports or in-home service providers.Document accordingly to assure compliance with MDHHS and other regulatory requirements.Document accordingly to assure funding and billing requirements are efficiently maintained.Perform other duties as assigned.Maintain regular and punctual attendance.Perform other duties as assigned by the supervisor or CEO consistent with the employees skills and Agency needs.

Education and/or Experience: Minimum of an Associate's degree in a social services field is preferred.

Will consider relevant experience.Minimum of two years Care Manager Experience.Experience in the health care system and the field of aging.Scheduling experience.Strong computer fundamentals and proficiency with Microsoft Office products.

Language Skills: Possess the ability to read, analyze and interpret information.

Possess the ability to respond to inquiries or complaints from staff and vendors.

Possess the ability to effectively present information to management and staff.
