LPN - Coummunity Liasion/Resource Linkage Coordinator

Schedule and Weekend Rotation Weekday shifts are 5:00 PM-1:00 AM.

The weekend assignment rotates every other week.

The employee assigned to the weekend works Monday-Wednesday 5:00 PM-1:00 AM, is off Thursday-Friday, and works Saturday-Sunday 8:00 AM-4:00 PM, totaling 40 hours.

The following week, the employee works Monday-Friday 5:00 PM-1:00 AM and is off Saturday-Sunday, totaling 40 hours.

The two LPN Coordinators alternate this pattern so weekend coverage flips biweekly.

General Position Description: The Community Liaison/Resource Linkage Coordinator LPN, directly reports to the Crisis Director, and serves as a dedicated after-hours intake, access, hospital liaison, referral triage and resource-linkage professional for Phoenix Counseling Center.

The position facilitates entry into services, works with hospitals and community organizations, identifies and coordinates appropriate resources, and connects individuals to Mobile Crisis Management, Facility-Based Crisis and other appropriate services.

The role supports qualifying CCP-RSP outreach, supportive contact, assessment/referral/resource linkage, community partnership and data/reporting activities.

Because this position requires active LPN licensure, providing direct Facility-Based Crisis (FBC) floor nursing coverage as needed is an essential function of the position.

The employee must be able and willing to perform assigned floor nursing responsibilities when operational, staffing, census, acuity or patient-care needs require, consistent with LPN scope of practice, demonstrated competency, authorized provider orders, required supervision and Phoenix Counseling Center policy.

Essential Job Duties and Responsibilities Facilitate entry on behalf of the program into hospitals, emergency departments, community organizations, referral networks and other community settings.

Serve as a liaison between Phoenix, hospitals, community providers, individuals/families and community partners.

Receive and respond to after-hours referrals; complete initial intake/access functions and timely triage of presenting needs.

Identify the most appropriate service/resource pathway and facilitate connection to Mobile Crisis Management, Facility-Based Crisis, other Phoenix programs or external community resources.

Provide intensive resource linkage for individuals and families who are navigating behavioral health, crisis, social-service or recovery-related needs.

Maintain current knowledge of community resources, referral mechanisms, eligibility considerations and points of access.

Maintain a closed-loop referral process by tracking referral receipt, outreach/contact, disposition, acceptance, arrival, denial/diversion, alternate linkage and follow-up.

Coordinate with FBC staff regarding available beds, pending referrals, anticipated admissions/discharges and barriers affecting bed flow.
