RN Clinical Program Coordinator - Sickle Cell Care

Work Schedule:

90% FTE.

Day shift.

You will four 9-hour shifts per week, Monday-Friday (day off to be determined).

No weekends, no holidays.

We're seeking a compassionate Registered Nurse to join our Adult Sickle Cell Disease Program.

In this role, you'll partner with a multidisciplinary team to deliver exceptional, patient-centered care while building meaningful relationships with patients and families.

We are seeking a RN Clinical Program Coordinator - Sickle Cell Care to:

Collaborate with a multidisciplinary team to provide high quality, compassionate and comprehensive care to adult patients with sickle cell disease.

Establish goals and strategies for meeting the continuing needs of patients.

Provide care coordination and follow up on care plans with a focus on symptom management.

Build trusting relationships and advocate for patients and families.

Help patients overcome barriers to care and connect with valuable resources.

Support program development through data collection, quality improvement, and outcomes evaluation.

Incorporate evidence-based practices and emerging advances into patient care.

Participate in community outreach and education to expand access and reduce stigma.

Qualifications

Bachelor's Degree in Nursing Preferred

Work Experience

2 years of relevant nursing experience Required

Experience in teaching patient and families Required

Licenses & Certifications

Registered Nurse, Licensed in the State of Wisconsin Required

Basic Life Savings (BLS) Upon Hire Required
