Public Health Planner

Under general direction, the Public Health Planner is responsible for planning and organizing public health programming at the local and regional community level.

This position will support further development of a Community Information Exchange (CIE) to enhance connectivity of organizations addressing health-related social care needs.

Working in partnership, this position will support improving access to healthcare by reducing barriers, increasing connectivity and supports the implementation of community health needs assessment andimplements strategies to promote active involvement of community members and stakeholders in developing and driving community health initiatives to promote individual and population health specific to Rural Health Transformation initiatives.

This position is in the office and community based work with travel across the 10 county jurisdiction required.
