JOB TITLE: Coach REPORTS TO: Service Coordinator JOB OVERVIEW: We are a growing organization serving an increasing number of families throughout the state of New Jersey.

We hire experienced professionals for a variety of part-time & full-time roles on a rotating basis throughout the year.

Our ideal candidates have a passion for serving and impacting the lives of adults with disabilities and their families.

We are currently seeking a reliable and experienced Coach to join our team.

In this role, you will be responsible for providing community-based supports with life skills and activities of daily living such as shopping, transportation to & from day programs and personal appointments, self-care management and meal preparation RESPONSIBILITIES & DUTIES: Responsible for providing direct support to assist participant(s) in daily routine, life skills & activities Assist participant in skill development to achieve stated goals and build independence as outlined in participants ISP and/or Face Sheet Ensure participant arrives to scheduled activities and appointments on time Assist with gross and/or fine motor skills including mobility issues as needed on uneven or unpaved walkways Document and submit activity reports within 24 hours of service(s) being provided in accordance with NJ State guidelines Punctuality, Professionalism & Reliability Are A Must!
