Physical Therapist PT Skilled Nursing

Sign on bonus: $5,000

The Emerald Peek Rehabilitation & Nursing Center in Peekskill, NY is a sub-acute facility with a full time Physical Therapist opening on our in house rehab team.

Hiring is done on site by our Director of Rehab and facility leadership, not by an outside recruiter.

You will interview with the people you will actually work with.

We are part of a growing network of 35 skilled nursing facilities, which means real clinical support behind you and real places to go from here.

Your Day to Day

* Complete physical therapy evaluations covering strength, range of motion, balance, gait, transfers, endurance, and fall risk

* Set functional, measurable goals tied to each resident's prior level of function and planned discharge setting, under physician direction

* Complete evaluations, weekly progress notes, recertifications, and discharge documentation accurately and on time

* Train residents, families, and caregivers on safe mobility, transfers, assistive device use, and home carryover

* Partner with nursing on safe transfer status, fall prevention, and restorative programming

What Makes This PT Job Different

* The whole team is in the room.

PT, OT, SLP, nursing, dietitian, respiratory, social work, and MDS build one plan together.

When a resident plateaus, you are not the only one looking at it.

* New graduates are trained, not thrown in.

Structured orientation, a named mentor, a gradual caseload ramp, and ongoing in house education from clinicians who want to teach.

If sub-acute is new to you, that is expected and planned for.

* Leadership that is present.

Our Director of Rehab and facility administration are invested in where you want to be in five years and will talk with you about it regularly, not only once a year at review time.

* Specialty tracks that are real roles, not titles.

A 35 facility network that is still expanding means program development, mentorship, and leadership paths, including:
+ Falls reduction program leads
+ Orthopedic and post surgical pathway champions
+ Cardiopulmonary and step down mobility specialists
+ Wheelchair seating and positioning clinicians
+ Assistant director and lead or senior therapist ladder programs

* Work life balance is an operating priority.

Tell us the schedule you need, and we will work with you to build it.

Our residents come first, and happy therapists equal happy patients.
