Community Health Coordinator

The Community Health Coordinator is responsible for advancing community-based substance use prevention, intervention, treatment, and recovery initiatives through strategic coordination, collaboration, and resource development.

This position serves as a central point of connection among community partners, service providers, and stakeholders to improve communication, strengthen service systems, and enhance access to resources for individuals and families.

The Coordinator supports the effective implementation of programs funded through Opioid Abatement and other available resources by promoting data-informed decision-making, identifying community needs, and fostering partnerships that improve community health outcomes.

This position works across multiple sectors to align prevention and intervention efforts, promote evidence-informed practices, and support a coordinated community response to substance use challenges.
