Community Waiver Coordinator

The Community Waiver Coordinator supports the day-to-day administration of Community Waiver Program services for an assigned caseload of people served, ensuring services align with each person's Person-Centered Plan and meet ADMH Community Waiver Program requirements.

This role coordinates documentation, scheduling, and communication among direct support staff, families, and support coordinators, and monitors service delivery for quality and compliance.

The Coordinator plays a key role in ensuring people served receive consistent, person-centered support across residential, day, and community settings.
