Field Clinical Assessment Coordinator (Hybrid Remote - Illinois)

Essential Duties and Responsibilities:
- Complete medical and/or behavioral health assessments that are typically routine using standardized tools and criteria in accordance with contract requirements; escalate complex or non-standard cases that fall outside defined protocols.
- Conduct on-site, video, and/or telephonic assessments based on program requirements to determine an individual's needs for services and supports, eligibility, and level of care based on established guidelines, seeking guidance from senior staff as needed.
- Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies; ensure accurate and consistent documentation of assessment outcomes.
- Travel to client, consumer or program locations to perform in-person assessments exceeding 50% of the workweek in support of program delivery.

- Conduct in-person, on-site outreach interviews and transitional assessments using person-centered and motivational interviewing techniques

- Educate individuals about their rights, the Outreach & Assessment process, and what to expect in next steps

- Accurately document assessment findings, outcomes, and required data elements in program systems

- Maintain professionalism and strong customer service when working with individuals with complex medical, behavioral health, and social needs

- Traveling as required based on program contract requirements.

- Perform other related duties as assigned.

Minimum Requirements

- High school diploma or GED required and 2+ years of relevant professional experience required, or equivalent combination of education and experience.
- Travel on-site to client, consumer or program locations for more than 50% per workweek required; specific travel requirements based on program contract requirements.

- Master's degree in counseling, social work, psychology, or another closely related field OR must have an active, unrestricted license in a relevant field, such as nursing, social work, psychology, counseling, occupational therapy, or another closely related discipline

- Minimum 5 years' experience in medical, mental health, or combination of both fields.

- Minimum of 5 years of experience conducting medical and/or mental health assessments

- Willingness and ability to travel up to 85% throughout the greater Chicago metropolitan area, with occasional travel outside the region based on business needs.

Preferred knowledge includes:

- Familiarity with Medicaid, Long-term services and supports, or community-based programs

- Strong interviewing, documentation, and interpersonal skills

- Experience working with individuals with complex medical or behavioral health needs

- Ability to work independently in the field and manage a mobile schedule
