Licensed Practical Nurse (LPN) - Float

The Licensed Practical Nurse (LPN) - Float (Jackson & Portsmouth), provides direct patient care and clinical support within a primary care setting to ensure high-quality, efficient, and patient-centered care.

Working as an integral member of the interdisciplinary healthcare team, the LPN supports providers through clinical workflows, patient triage, medication processes, documentation, and care coordination while maintaining compliance with clinical, safety, and regulatory standards.

The purpose of this role is to positively impact patient well-being through excellent clinical care, effective communication, and collaboration with the healthcare team.

Morning Huddle and Pre-Visit Planning Estimated Effort: 10% Prepare Pre-Visit Planning (PVP) for assigned provider and primary care team.

Review and ensure PVP is completed accurately each morning.

Arrive on time and demonstrate a pleasant demeanor, positive attitude, and team-oriented approach.

Patient Triage (As Assigned) Estimated Effort: 5% Answer live patient calls for assigned primary care team as directed by the Lead Nurse or Clinical Services Manager.

Provide excellent customer service and clear, professional communication.

Collaborate with providers and team members to respond to patient questions and requests appropriately.

Medication Prior Authorizations (As Assigned) Estimated Effort: 5% Complete medication prior authorizations for assigned primary care teams as directed by the Lead Nurse or Clinical Services Manager.

Communicate with patients regarding the status of prior authorizations and next steps.

Attend meetings, conferences, and training sessions related to assigned duties.

Direct Patient Care Estimated Effort: 75% Provide excellent direct patient care in accordance with clinic workflows and nursing scope of practice.

Collaborate with assigned providers to support quality, efficient patient care.

Complete daily clinical tasks as assigned.

Administer immunizations as ordered.

Perform point-of-care testing as ordered.

Complete all EHR documentation accurately and in a timely manner.

Maintain a safe work environment and adhere to all clinical, compliance, infection control, and safety policies and procedures.

Service and Other Responsibilities Estimated Effort: 5% Complete additional tasks as assigned by the Clinical Services Manager or Director of Nursing and Clinical Operations.

Minimum Requirements Education: High School Diploma or equivalent.

Job-Related Experience: Experience working in an office or primary care clinical setting.

People Management Experience: Not required.

Licenses, Certifications, and Accreditations: Current Licensed Practical Nurse (LPN/LVN) license Current CPR certification Preferred Qualifications One (1) to three (3) years of primary care experience Working Conditions This position operates in a climate-controlled clinical environment.

Duties require sitting, standing, walking, bending, reaching, and frequent use of hands for clinical tasks.
