Community Outreach & Engagement Manager

Who You Are: Amoskeag Health is seeking a Community Outreach & Engagement Manager to lead wellness, prevention, and education initiatives across Manchester and Greater Manchester.
This role manages contract-specific programs aligned with Amoskeag Healths Strategic Plan, collaborates with internal leadership and external partners, and facilitates community stakeholder groups focused on prevention and population health.
The ideal candidate is a strategic thinker and relationship-builder, passionate about improving community wellness and driving impactful, scalable initiatives.
What You'll Do: Supervise program staff to include hiring, training, coaching, mentoring, and evaluation.
Maintain a competent and engaged staff by communicating regularly, identifying training needs, completing performance appraisals, providing coaching or corrective counseling, and providing recognition of strong performance.
Staff to include Navigators, Newborn Navigator, Youth Health Education program staff, ACERT Supervisor, and any others appliable to new initiatives and programs.Monitor direct reports achievement of functional excellence, accomplishment of strategic and operational objectives to achieve desired outcomes.
Identify best practices and evidence-based methodologies for the delivery of services, assuring staff are appropriately trained in utilization of those practices and methodologies.
Audits charts to assess utilization of those practices, methodologies, mentoring, when needed, and to ensure quality and timely documentation.Work with the staff to provide analytical data to critically assess the impact of community outreach, education, and other service/engagement interventions following commonly used quality metrics and those determined by the Amoskeag Health Strategic Plan and any other related work plans.Manage planning and implementation activities with other local social service providing agencies related to the population of focus and wellness/prevention strategy.Communicate regularly with appropriate private foundations, federal departments, State of NH departments and divisions, and local department stakeholders and participate in relevant committees, as needed.Facilitate/co-facilitate and support the oversight of the following: Manchester LAUNCH Family Wellness CollaborativeAdditional collaborative, as assigned.Responsibilities and activities for these collaboratives include:Development of relevant agenda items associated with promotion, prevention, and integration of wellness/prevention activities.Coordination of local level work with the state level infrastructure and policy development.Development of mutual planning and implementation strategies.Assist in program implementation as identified by the collaboratives.Address issues necessary to resolve local barriers.Work to continually recruit, engage and support members.Resp...
