Community Health Worker

Location: Turlock, CA

Department: Family & Community

Reports to: Program Supervisor

Schedule: Full-Time / Non- Exempt Monday-Friday 8AM-5PM, occasional evening and weekends as needed.

Travel: Field-based role with regular local travel (approximately 50-70%)

Pay Range: $24.00 - $24.50/ hr

The hourly range for this position is $24.00 - $24.50/ hour for employees hired in the Central Valley.

Position Summary
The Community Health Worker (CHW) provides outreach, health education, care coordination, and advocacy to Medi-Cal members and community members with complex needs.

This is a highly field-based role, requiring strong relationship-building skills and the ability to navigate healthcare and social service systems effectively.

Key Responsibilities

Community Outreach & Engagement


* Conduct home visits, field outreach, and community-based engagement


* Provide culturally responsive health education (mental health, substance use, chronic disease, wellness)


* Lead or support community workshops, outreach events, and education campaigns


* Conduct screenings to identify social determinants of health (SDOH) needs

Care Coordination & Navigation


* Help individuals navigate healthcare, behavioral health, and social service systems


* Connect clients to housing, benefits, food access, and other critical resources


* Coordinate with providers, case managers, and community partners


* Support engagement in treatment plans and long-term care goals

Advocacy & Relationship Building
