Care Manager - L.I.N.K. Program

Essential Duties & Responsibilities Achieve primary L.I.N.K.

program goal to prevent suicide, substance use escalation, and future behavioral health crises.

Contact referred individuals within 24 hours of referral or care transition.

Complete intake assessments to identify participant needs, goals, strengths, and barriers to care.

Obtain and document participant consent for follow-up services.

Provide weekly follow-up contacts and supportive outreach for a minimum of 90 days, with increased contact as needed.

Conduct ongoing suicide and substance use risk assessments and assist with safety planning.

Educate participants about available behavioral health, substance use, and community resources and provide appropriate referrals.

Facilitate connections to recommended services and support participants in overcoming barriers to engagement.

Coordinate care with hospitals, crisis services, community providers, case managers, therapists, family members, and other natural supports, as appropriate.

Monitor participant progress and reassess goals, needs, and risk factors on an ongoing basis.

Respond to participant requests for additional support and intervention.

Maintain accurate, timely, and HIPAA-compliant documentation of all contacts, assessments, referrals, and outcomes.

Participate in crisis intervention, de-escalation, and care coordination efforts to promote ongoing safety, stability, and recovery.

Attend meetings and trainings with L.I.N.K.

team, department, agency all-staff events, and community partners, as assigned.

Prepare and conduct educational presentations about the L.I.N.K.

program for potential referral sources Develop and foster strong, positive relationships with referral sources to identify individuals who could benefit from behavioral health interventions and connect them with care.

Work Conditions and Environment This is a hybrid position which allows the employee to work in the crisis room at Providents St.

Louis City office (2650 Olive Street, St.

Louis, MO 63103) or from home.

Position is part time, with hours falling within L.I.N.K.

program operating hours (between 9:00 am and 7:00 pm, 7 days a week).

Specific schedule to be developed in coordination with supervisor.

Must have a reliable high-speed internet connection and quiet, confidential workspace to work remotely.

Have the ability to sit for long periods of time, making and receiving calls and video conferencing appointments using computer & headset.

Must have stamina to provide live, in person presentations in small to large group settings for 30-60+ minutes.

Must have reliable transportation to travel to Providents offices and partner agency locations for meetings, trainings, and presentations
