Hayward Gardens Post Acute is seeking a compassionate, motivated, and skilled Physical Therapist Assistant (PTA) to join our growing rehabilitation team on a Full-Time basis.
Under the direction and supervision of a licensed Physical Therapist, the PTA will provide hands-on therapy services designed to help residents improve mobility, strength, balance, coordination, and overall functional independence.

Position Summary
The Physical Therapist Assistant (PTA) assists the Physical Therapist in implementing individualized treatment plans and providing quality, resident-centered therapy services.
The PTA works closely with residents, families, nursing staff, and the interdisciplinary team to promote optimal rehabilitation outcomes and quality of life.
Essential Responsibilities
* Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
* Assist residents with exercises and therapeutic activities designed to improve strength, mobility, balance, coordination, and endurance.
* Assist residents with transfers, ambulation, positioning, and functional mobility.
* Monitor resident response to treatment and communicate progress or concerns to the supervising Physical Therapist.
* Accurately document therapy services and resident progress in accordance with facility, state, federal, and payer requirements.
* Educate residents and caregivers on exercises, mobility techniques, safety strategies, and fall prevention.
* Assist with recommendations and training related to appropriate adaptive and mobility equipment.
* Maintain a safe, clean, and organized therapy environment.
* Follow infection control, safety, and facility policies and procedures.
* Participate in interdisciplinary meetings, care conferences, and rehabilitation team activities as needed.
* Collaborate with nursing and other healthcare professionals to support resident care goals.
* Participate in quality assurance and performance improvement initiatives.
* Maintain confidentiality of resident and company information.
* Perform other duties as assigned by rehabilitation leadership.
Qualifications
* Current California Physical Therapist Assistant (PTA) license in good standing.
* Graduate of an accredited Physical Therapist Assistant program.
* Strong communication and interpersonal skills.
* Compassionate, professional, and resident-centered approach to care.
* Ability to work effectively as part of an interdisciplinary team.
* Skilled nursing or post-acute rehabilitation experience preferred.
