San Bruno Skilled Nursing is seeking a knowledgeable and detail-oriented MDS Nurse (LVN) to join our team on a part-time basis.

* Pay Rate: $50k-60k (DOE)

Key Responsibilities


* Coordinate and complete MDS assessments, Care Area Assessments (CAAs), and related documentation within required timelines.


* Ensure clinical documentation accurately reflects resident conditions and supports reimbursement processes.


* Maintain compliance with CMS, state, and facility regulations.


* Collaborate with the interdisciplinary team to develop and update individualized care plans.


* Monitor resident progress and ensure care plan revisions are completed as needed.


* Participate in care conferences and quality assurance activities.


* Ensure timely and accurate MDS submissions for Medicare and Medicaid reimbursement.


* Assist during audits, surveys, and regulatory reviews.


* Provide education and guidance to staff regarding documentation standards and MDS requirements.


* Communicate effectively with residents, families, and healthcare team members regarding care planning and resident needs.


* Stay informed of regulatory updates affecting MDS processes and skilled nursing documentation.

Qualifications


* Current California LVN license in good standing.


* Previous experience in skilled nursing, post-acute care, or long-term care preferred.


* Working knowledge of MDS 3.0, the RAI process, and Medicare/Medicaid reimbursement guidelines.


* Strong organizational, analytical, and documentation skills.


* Ability to manage multiple deadlines while maintaining accuracy and compliance.


* Experience with electronic health record (EHR) systems.


* Proficiency in Microsoft Office applications.


* Strong communication and collaboration skills.
