Responsibilities


* Perform comprehensive physical therapy evaluations and develop individualized treatment plans.


* Provide skilled physical therapy services based on residents' needs and goals.


* Implement evidence-based interventions to improve strength, mobility, balance, endurance, and functional independence.


* Monitor resident progress and adjust treatment plans as appropriate.


* Educate residents and families regarding therapy goals, exercises, mobility, and safety.


* Collaborate with PTAs and the interdisciplinary care team to support positive clinical outcomes.


* Maintain accurate and timely clinical documentation.


* Promote resident safety, dignity, independence, and quality of life.


* Participate in care planning and interdisciplinary team activities as needed.

Qualifications


* Current California Physical Therapist (PT) License in good standing required.


* Strong communication and interpersonal skills.


* Compassionate, resident-centered approach to care.


* Ability to work effectively as part of an interdisciplinary team.
