* Location: Vasona Creek Healthcare - Los Gatos/South Bay Area


* Pay Rate: $57.00-$65.00+ per hour, depending on experience


* Opportunities: Full-Time, Part-Time & PRN

Your Role & Impact

As a Physical Therapist, you will play an essential role in helping residents achieve their highest level of function and independence.

Responsibilities include:


* Perform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.


* Provide skilled therapy services to short-term rehabilitation and long-term care residents.


* Treat patients recovering from joint replacements, cardiac events, strokes, and other medical conditions.


* Utilize specialized rehabilitation programs, including Acute Neuro Rehabilitation and Pain Management .


* Monitor patient progress and modify treatment plans as appropriate.


* Collaborate with PTAs, OTs, SLPs, nursing, and other members of the interdisciplinary team.


* Educate patients and families regarding treatment plans, exercises, mobility, and strategies for continued progress.


* Promote safe mobility, functional independence, and successful discharge planning.
