Senior Manager, Population Health Project, Programs & Grants

JOB ESSENTIAL DUTIES: Project and Program Leadership Lead the planning, development, implementation, and evaluation of population health projects and strategic initiatives.Develop project plans, timelines, budgets, performance metrics, and operational workflows.Oversee multiple concurrent projects while ensuring deliverables are completed on time and within budget.Evaluate program effectiveness and develop recommendations for continuous improvement.Utilize data analytics and population health tools to identify opportunities to improve patient outcomes and operational efficiency.Ensure alignment of projects with organizational strategic goals and regulatory requirements.Present project updates, outcomes, and recommendations to executive leadership and key stakeholders.

Mammogram Bus Program Leadership Provide operational oversight and strategic direction for the organization's Mammogram Bus Program.Coordinate mobile mammography services, community outreach events, and screening initiatives.Develop partnerships with community organizations, employers, schools, faith-based organizations, and local agencies to increase access to breast cancer screening services.Monitor program utilization, patient engagement, screening outcomes, and operational performance.Identify opportunities to expand mobile health services and improve access to preventive healthcare for underserved populations.Ensure compliance with all regulatory, clinical, and grant requirements related to mobile screening operations.Track and report program performance metrics and community impact outcomes.

Grant Management Research, identify, and pursue federal, state, local, and private grant funding opportunities.Lead the development, writing, and submission of grant proposals and applications.Manage awarded grants from implementation through closeout.Ensure compliance with grant requirements, deliverables, reporting obligations, and budgetary guidelines.Monitor grant expenditures and collaborate with finance teams to maintain fiscal accountability.Prepare grant reports, outcome analyses, and funding updates for executive leadership and funding agencies.Build and maintain relationships with grantors, funding organizations, and community stakeholders.
