Staff Nurse (LPN)

POSITION SUMMARY: The Licensed Practical Nurse (LPN) - Population Health supports the delivery of proactive, patient-centered care by assisting with care coordination, chronic disease management, preventive health initiatives, and quality improvement activities.

Working as part of a multidisciplinary team, the LPN engages patients through outreach, education, and follow-up to improve health outcomes, close care gaps, and promote adherence to treatment plans.

The LPN plays a key role in supporting value-based care initiatives, preventive screenings, care transitions, and ongoing management of patients with chronic conditions.
