Key Responsibilities


* Evaluate patients and develop individualized treatment plans.


* Provide skilled physical therapy services in accordance with physician orders and care plans.


* Supervise Physical Therapist Assistants (PTAs) in compliance with state practice regulations.


* Collaborate with interdisciplinary team members to coordinate and optimize patient care.


* Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.


* Maintain timely and accurate documentation, including evaluations, treatment notes, progress reports, and discharge summaries.


* Educate patients, families, and caregivers on treatment goals, mobility strategies, safety techniques, and home exercise programs.


* Assist with discharge planning and recommendations for continued care.
