Victorian Post Acute is a beautiful 90-bed, rehabilitation-focused skilled nursing facility dedicated to providing outstanding care for both short-term rehabilitation and long-term care residents.
We are currently seeking a compassionate and motivated Occupational Therapist (OTR) to join our growing in-house therapy team.

Key Responsibilities
* Evaluate residents and develop individualized occupational therapy treatment plans.
* Provide skilled occupational therapy services in accordance with the established plan of care.
* Supervise Certified Occupational Therapy Assistants (COTAs) in compliance with California practice guidelines.
* Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to achieve optimal outcomes.
* Participate in care conferences, rehabilitation meetings, and discharge planning.
* Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
