Registered Dietitian (RD)
General Purpose
The Registered Dietitian is responsible for assessing residents' nutritional needs and developing individualized nutrition interventions that support their health, recovery, and overall quality of life.
This position collaborates with nursing, dietary services, physicians, rehabilitation, and other members of the interdisciplinary team to ensure residents receive appropriate nutritional care in accordance with physician orders, facility policies, and applicable federal and state regulations.
Essential Duties and Responsibilities
* Complete comprehensive nutritional assessments for new admissions, quarterly reviews, annual assessments, significant changes, and residents identified as nutritionally at risk.
* Develop individualized nutrition care plans based on each resident's medical condition, dietary requirements, preferences, allergies, and cultural needs.
* Monitor residents' nutritional status, food intake, hydration, laboratory results, skin integrity, and weight trends.
* Evaluate significant weight changes and recommend appropriate nutritional interventions.
* Complete required MDS documentation, progress notes, care plans, and nutritional assessments accurately and within established timeframes.
* Recommend therapeutic diets, nutritional supplements, fortified foods, tube-feeding regimens, and other interventions as appropriate.
* Collaborate with physicians and the interdisciplinary team regarding diet orders and nutritional recommendations.
* Participate in care conferences, weight-management meetings, quality assurance activities, and other clinical meetings as required.
* Provide nutrition education and counseling to residents, families, and staff.
* Review menus to ensure meals meet residents' nutritional needs and applicable regulatory standards.
* Monitor the preparation and service of therapeutic diets, altered-texture meals, supplements, and tube feedings.
* Conduct routine kitchen and dining-service observations to evaluate food quality, portion sizes, sanitation, meal temperatures, and resident satisfaction.
* Work closely with the Dietary Director and culinary team to address resident preferences and improve the dining experience.
* Assist with survey preparation and participate in regulatory inspections as needed.
* Maintain resident confidentiality and protect all health information in accordance with HIPAA and facility policies.
* Report changes in residents' nutritional status and other concerns to the appropriate clinical team members.
* Follow infection prevention, food safety, and workplace safety standards.
* Perform additional duties as assigned.
Qualifications
* Bachelor's degree or higher in dietetics, nutrition, food science, or a related field from an accredited educational institution.
