Care Management Coordinator (Yiddish Speaking)

Tracking preventative care requests.

Works with members and Care Managers to connect members to preventative care providers and schedule appointmentsRegistering members for vaccine appointments in community and homeTroubleshooting issues member experience with medical providers and appointmentsCoordinating care management mailingsProvides administration and department support for care management team.

Assists in completing activities related to different projects/initiatives in support of departmental goals.Maintains and supports documentation, logs and data entry to monitor activity and/or outcomes in support of departmental goals as needed.Back-up to the Member Service/Transportation department and offer coverage/assistance as needed.Performs other duties as assigned.

Specific Knowledge, Skills, and Abilities: Proficient in the use of office equipment and the computer; Microsoft Word and Excel.Self-directing, good organizational and interpersonal skills.

Required Education, Experience, and Licenses: Graduate of high school.

1 year satisfactory office experience; experience in Health Care, desired.

Bilingual preferred (Bengali, Hindi, Urdu, Yiddish).
