(Paid 50% at 1 month and 50% at 6 months)

Position Summary

As a Peer Navigator in Modesto, California, you will be assisting clients and their families in gaining access to primary preventative and specialty health care services.

You will be providing specific assistance with medication management, housing, counseling, support and advocacy.

Additionally, responsible for engaging, educating, and offering support to individuals and their family in order to successfully connect them to culturally relevant health services including prevention, diagnosis, timely treatment, recovery management, and follow-up.

* Provides emotional support to alleviate fears of and barriers to accessing quality health care.


* Assists assigned clients to identify and progress toward achievement of their chosen goals.


* Motivate and educate individuals/families/caregivers about the importance of preventative services, medical conditions and recovery strategies.


* Advocates for clients in all areas of treatment and helps them apply for and receive services.


* Empowers clients in meeting their goals.
