Hospice Triage Coordinator RN Department: Hospice Pay Range: $43.00 - $46.00/Hour Position Type:Full Time Schedule: Monday - Friday 8- 4:30 Location:In Office - Rochester, NH Position Summary Cornerstone VNA is seeking an experienced and compassionate Registered Nurse (RN) to join our Hospice team as a Hospice Triage Coordinator.

The Hospice Triage Coordinator serves as a central point of communication and coordination for our hospice program.

This position manages incoming patient and caregiver calls, coordinates hospice referrals and admissions, supports patients and families during times of crisis, and helps ensure information is communicated effectively among the hospice team, referral sources, physicians, and other departments.

This role is ideal for an experienced hospice nurse who is highly organized, comfortable managing multiple priorities, and able to remain calm and supportive when assisting patients and families with urgent or sensitive needs.

What You'll Do Triage incoming calls from hospice patients and caregivers and coordinate with appropriate team members to address patient needs.Provide calm, compassionate guidance to patients and caregivers, including support related to medication administration and emotional needs.Document triage calls and clinical information accurately and timely.Coordinate hospice referrals and collaborate with referral sources and interdisciplinary team members to support timely admissions.Track and follow up on pending and potential hospice referrals.Coordinate completion of hospice admission consents.Assist with securing respite and General Inpatient (GIP) beds, including medical records, reports, and transportation coordination.Assign care teams for new hospice admissions.Coordinate communication between hospice leadership, field staff, referral sources, Health Information, and physician offices.Communicate changes in orders, scheduling, and patient needs to appropriate personnel.Assist with hospice EMR workflows and other administrative or clinical coordination needs.Support additional nursing functions as needed, including hospice support and GIP visits.Utilize technology, including virtual nursing tools, to enhance patient care and support the hospice team.Communicate staffing concerns, patient-care issues, and other significant needs to the Hospice and Palliative Care Director.

What We're Looking For Current Registered Nurse (RN) license in the State of New Hampshire, Maine or another compact state.Hospice Experience PreferredStrong clinical judgment and problem-solving skills.Excellent verbal and written communication skills.Ability to remain calm, professional, and compassionate while assisting patients and families during stressful situations.
