Division/Department/Program: Southside AGR (IDD) The Residential Care Provider monitors general resident activity to observe and report behavior/progress of individuals with severe developmental disabilities living in a group home or other community residential setting and provides individualized skills training according to residents needs.

Under direct supervision, assists with daily living activities in a residential facility.

Provides training of basic personal and social skills, simple academics, or work skills to consumers/students in a state group home, workshop, or supported employment environment.

Duties & Responsibilities: Assist individuals with Developmental Disabilities in basic daily living skills.Carry out a person-centered Individual Service Plan (ISP).Ensure individuals are dressed neatly and appropriately and are always groomed according to VPH policy.Assists and encourages individuals to care for immediate personal needs such as toileting, washing their hands, and eating.Teaches individuals daily skills such as household chores, laundry, cleaning, and washing dishes, as appropriate according to their individual plan.Transports individuals to work, day programs, doctor appointments, shopping, recreational activities.Assist with scheduling medical appointments.Assists in the implementation of individual and behavioral plans and provides feedback for plan goals.Assists individuals in the pursuit of recreational, social and community activities based on their preferences and individualized treatment plans.Assists and encourages individuals in the establishment of social networks.Promotes participation and involvement in community outings.Ensure everyones rights are protected.Treats individuals with dignity and respectMaintains the residence in a clean and organized condition.Other related duties as assigned.

Minimum Qualifications: High school diploma/GED and six (6) months of relevant job-related training or experience.

Preferred Qualifications: Experience working with IDD population.

Certified Nursing Assistant certification (CNA), Direct Support Professional (DSP), Direct Care Professional (DCP) or Patient Care Technician (PCT) preferred.

Requirements/Competencies: Must pass a pre-employment drug screen and subject to random drug screens.Must have a valid Drivers License and a clean Motor Vehicle Record in accordance with company policy.Must pass a criminal background check (including fingerprinting).Requires TB Evaluation at pre-employment and annually.Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have the following technical skills Word, Excel and Electronic Health Record (Care Logic).Must be willing to transport residents in the company van.M...
