View Point Health Job Title: Residential Care Provider (RCP) Job Code: SSS010/ Client Support Worker or T6001/Support Services Worker Shift: FT 1st Shift/3rd Shift PRN Job Hours:Hours May Vary (40 hours per week on average) Base Location: Conyers, GA.

Division/Department/Program: Eastside ITR The Residential Care Provider monitors general resident activity to observe and report behavior/progress of individuals with severe developmental disabilities or mental illnesses living in a group home or other community residential setting and provides individualized skills trainings according to residents needs.

Duties & Responsibilities: Determines the individuals strengths, needs, abilities, and preferences by observingdaily activities and reviewing records.Assists individuals in basic problem-solving and daily decision-making using the least intrusive methods appropriate for each.Provides training and oversight in daily living skills and personal care, such as dressing, hygiene, housekeeping, cooking, laundry, money management and socialization skills using techniques appropriate for their abilities.Maintains the residence in a clean and organized condition.Plans, prepares, and serves balanced meals.Transports individuals to work, day programs, doctor appointments, shopping, recreational and other activities.Supervises the self-administration of individuals medications using agency protocols.

Maintains require records.Document individuals progress toward goals and objectives identified in their treatment and/or behavior plans through activity/progress notes, data-tracking sheets, and/or other prescribed skill builder formats.Complete all home and vehicle records, logs, notes, assessments, reports,and other documentation according to protocols.Lifts and carries groceries and supplies and move furniture for cleaning.

Assists disabled individuals on/offvehicle and secures seatbelts.

Performs other duties that require moderate lifting.

May use a gait belt or mechanical lift to lift and position individuals, as needed.

Other duties as assigned.

Minimum Qualifications: High school diploma or GED AND any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.

Preferred Qualifications: Experience working with IDD population.

Certified Nursing Assistant certification (CNA), Direct Support Professional (DSP), Direct Care Professional (DCP) or Patient Care Technician (PCT) preferred.

Requirements/Competencies: Must pass a pre-employment drug screen and subject to random drug screens.Must have a valid Drivers License and a clean Motor Vehicle Record in accordance with company policy.Must pass a criminal background check (including fingerprinting).Requires TB Evaluation at pre-employment and annually.Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have the following technical skills Word, Excel and Electronic Health Record (Care Logic).Must be willing to...
