Behavioral Health Project Coordinator
POSITION SUMMARY The Project Coordinator is responsible for day-to-day coordination of grant activities and serves as an integral part of the Behavioral Team.
The position will be responsible for overall activities and data collection and ensuring timely follow-up with new and existing referrals that are related to a federal grant.

ESSENTIAL DUTIES AND RESPONSIBILITIES Grant Management Serves as the project coordinator for a federally awarded grant and provides the day-to-day operation support.
Monitors grant awards and works with Director of Behavioral Health on ensuring spend down of grants.
Completes all documents, forms, or reports required by the grant with support of the Grants Team.
Coordinates monitoring of program outcomes and compiles data.
Maintains accurate records and collection of data, which may include, rosters, meeting notes, survey results, patient outcomes, etc.
Prepares presentation materials for meetings, recruitment, and training on the federal program.
Behavioral Health Accepts client referrals for behavioral health services within the federal program.
Documents client interactions thoroughly in a timely manner.
Advocates, assesses, arranges, links, and/or integrates multiple services for clients.
Participates or hosts community outreach events.
MINIMUM REQUIREMENTS Associates degree in health sciences or related fieldOne year of experience in a social services or healthcare related field Demonstrated experience serving a diverse population PREFERRED REQUIREMENTS Bachelors degree in health sciences or related field.
Lived experience with mental health or substance use disorders.
Experience working with individuals with mental health and a history of substance use Experience facilitating groups and leading meetings Knowledge of case management and rehabilitation methods, principles, and techniques as they relate to mental health and substance use disorder t...
