Licensed Practical Nurse ICF Residential Services Are you looking for a nursing role where you can build relationships, provide hands-on support, and make a meaningful difference in the lives of individuals with intellectual and developmental disabilities? Valley Community Services Board is seeking a Licensed Practical Nurse (LPN) to provide nursing services to individuals with intellectual and developmental disabilities living in two residential group homes.

In this role, you'll help support the health and well-being of the individuals we serve while working closely with physicians, Direct Support Professionals, and other members of the support team.

What You'll Do As an LPN supporting our residential services, you'll help ensure individuals receive safe, coordinated healthcare while working collaboratively with their healthcare providers and residential support teams.

Responsibilities include: Provide nursing services in consultation with psychiatrists, primary care physicians, Direct Support Professionals, and other members of the support team.Assess individuals' physical health status at least quarterly and identify changes or concerns requiring follow-up.Review daily progress notes and documentation related to medications, medical appointments, and medical procedures.Oversee medication administration and the inventory, storage, acquisition, and management of medications, medical supplies, and equipment.Provide medication administration training, guidance, and ongoing support to ICF team members.Work collaboratively with residential staff, healthcare providers, and other partners to support individuals' ongoing healthcare needs.Develop and maintain effective working relationships with team members and external healthcare partners.
