ACT Team BHT

The ACT Team BHT is responsible for providing high quality care in the outpatient setting in which they work.

The BHT is responsible for utilizing good skills in assessment, treatment planning, case management, and the documentation of services.

The BHT is expected to function well as a member of the Clinical Team.

Responsible for providing targeted case management services and assessments, service planning and evaluation, linking, advocacy, and crisis intervention to assigned case load.
