Moraga Post Acute
PRN Position | Starting Rate: $58-$63+/hour

Moraga Post Acute is seeking a compassionate and skilled Occupational Therapist (OTR) to join our rehabilitation team on a PRN basis.

Position Responsibilities


* Evaluate residents and develop individualized treatment plans based on functional needs and physician orders.


* Provide skilled occupational therapy services focused on improving independence with activities of daily living (ADLs).


* Supervise and collaborate with Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.


* Work closely with physicians, nursing staff, residents, families, and interdisciplinary team members to coordinate care and optimize outcomes.


* Participate in care conferences, rehabilitation meetings, and discharge planning.


* Educate residents and caregivers on treatment goals, home exercise programs, adaptive techniques, safety awareness, and fall prevention.


* Recommend adaptive equipment and durable medical equipment (DME) to support resident independence and successful transitions.


* Maintain accurate, timely, and compliant clinical documentation.


* Contribute to quality improvement initiatives and staff education programs.

Qualifications


* Current California Occupational Therapist (OTR) License required.


* Strong clinical, communication, and organizational skills.


* Ability to work effectively within a multidisciplinary team.
