Medical Hill Healthcare is seeking a compassionate, dependable, and clinically skilled Licensed Vocational Nurse (LVN) to join our nursing team on an on-call/per diem basis.

This is an excellent opportunity for an LVN looking for flexible scheduling while making a meaningful difference in the lives of our residents.

The LVN will provide direct nursing care, support resident safety and comfort, and work collaboratively with CNAs, RNs, physicians, and the interdisciplinary team.

Position Responsibilities


* Provide direct nursing care to residents according to individualized care plans


* Administer medications and treatments as ordered


* Monitor residents' conditions and promptly report changes to the RN or physician


* Perform nursing assessments and accurately document resident care


* Assist with admissions, transfers, and discharges as needed


* Communicate effectively with residents, families, and members of the interdisciplinary team


* Provide wound care and other treatments within the LVN scope of practice


* Monitor vital signs and other clinical indicators


* Maintain accurate and timely medical records


* Follow infection prevention and control procedures


* Respond promptly to resident needs, emergencies, and call lights


* Ensure compliance with facility policies, state and federal regulations, and standards of nursing practice


* Promote resident dignity, safety, comfort, and quality of life

Qualifications


* Current and active California LVN license required


* Current CPR certification required or preferred


* Previous skilled nursing, post-acute, or long-term care experience preferred


* Strong clinical assessment and communication skills


* Ability to work independently and effectively as part of a team


* Dependable, compassionate, and resident-focused


* Strong organizational and time-management skills


* Ability to work flexible on-call shifts as needed
