Occupational Therapist Responsibilities


* Perform occupational therapy evaluations and assessments


* Develop individualized treatment plans based on resident goals and clinical needs


* Implement therapeutic interventions to improve mobility, strength, coordination, cognition, and activities of daily living (ADLs)


* Assist residents in regaining independence and maximizing functional abilities


* Educate residents, families, and caregivers on treatment plans and adaptive techniques


* Recommend and train residents on adaptive equipment and environmental modifications


* Collaborate with physicians, nursing staff, therapy team members, and interdisciplinary care teams


* Maintain timely and accurate clinical documentation in compliance with facility, state, and federal requirements


* Monitor resident progress and modify treatment plans as appropriate


* Participate in care conferences and discharge planning

Qualifications
Required


* Active and unrestricted California Occupational Therapist (OT) License


* Graduate of an accredited Occupational Therapy program


* Strong clinical assessment and treatment skills


* Excellent communication, documentation, and interpersonal skills


* Ability to work independently and collaboratively within a multidisciplinary team

Preferred


* Experience in Skilled Nursing Facilities (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care


* Knowledge of Medicare, PDPM, and rehabilitation documentation practices
