Responsibilities

As a Physical Therapist, you will:


* Evaluate residents and develop individualized treatment plans


* Provide skilled physical therapy services in a skilled nursing and post-acute setting


* Improve patient mobility, strength, balance, endurance, and functional independence


* Monitor patient progress and modify treatment plans as appropriate


* Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals


* Educate patients and families regarding treatment goals, recovery expectations, and home exercise programs


* Complete accurate and timely clinical documentation in compliance with state and federal regulations


* Contribute to positive patient outcomes and quality-of-care initiatives

Qualifications


* Current and unrestricted California Physical Therapist License required


* Graduate of an accredited Physical Therapy program


* Skilled Nursing Facility (SNF), rehabilitation, long-term care, or post-acute experience preferred


* Strong clinical assessment and treatment skills


* Excellent communication, organization, and time-management abilities


* Ability to work independently while contributing to a team-focused environment


* Commitment to delivering patient-centered care
