Admissions Coordinator
Admissions Coordinator ClarkLindsey, a premier Community serving older adults, located in Urbana, IL, is seeking a Skilled Care Admissions Coordinator.
This position is essential in managing the admissions process for both short-term rehabilitation and long-term skilled nursing care residents at MeadowBrook and the Green House Homes.
This position also facilitates care plans, as well as identifies and supports resident psychosocial and emotional health needs.

Experience in skilled care admissions, working with insurance companies, ADS assessments, discharge planning, and commitment to fostering a holistic and person-centered approach to resident wellness.
Key Responsibilities Admissions & Care Planning Serve as the primary contact for prospective residents and families.
Manage referrals, pre-admission assessments, and the full admissions process.
Collaborate with others to create and update resident care plans.
Participate in interdisciplinary team meetings to review progress and plan discharges.
Coordinate insurance notifications, appeals, and communication.
Complete and submit MDS assessments according to the requlatory requirements.
Grievance Officer Designee Document and track resident concerns.
Assist with investigations and ensure confidentiality.
Ensure timely and fair resolution of grievances.
Resident & Family Council Coordination Facilitate and document monthly Resident Council meetings.
Organize quarterly Family Council meetings.
Serve as liaison for follow-up to concerns.
Psychosocial & Emotional Health Support Conduct assessments and provide emotional support to residents.
Refer residents to external providers for longer-term mental health support.
Lead support groups and educational sessions.
Resident Services Team Collaboration Guide residents and families through care transitions.
Provide cross-functional support to Resident Services roles across campus.

Qualifications Required: 2+ years in senior living or healthcare.
Proficiency in MDS 3.0 and Medicare regulations.
Experience in the responsibilities listed above.
Strong interpersonal, organizational and communication skills are important.
Preferred: Knowledge of skilled care admissions regulations.
Experience in fast-paced interdisciplinary settings.
Compensation & Benefits The pay range for this position is based on experience.
$24.65/hr - $27.20/hr.
Bonuses: Up to 2% of wages, annually.
