SOCIAL WORKER - WEEKENDS
The position implements and is responsible for the delivery and overall functions of social services.
Social services are provided according to WTRC policy as well as accepted standards and practice acts.
Responsible for planning and directing a social work program.
Performs social services work with patients and families.
Criteria is followed for referrals from members of IDT.
Case identification for high-risk admissions population is in place and followed.
Direct services for social, environmental, and emotional problems for patients and their families.
Sharing in assessment, admission, service availability, treatment plan design, implementation of services, review of services, and evaluation responsibilities of the IDT.
Assess the current, personal, social, economic, and emotional needs of the patients.
Evaluates the family situation as it relates to rehabilitation plans (composition, significant dynamics).
Provides support, guidance, and psychological casework or group work as needed.
Interprets social, emotional, and family problems to coworkers and aids communication between patients, family, and coworkers as appropriate.
Coordinates psycho-social needs and services with other patients/family and services being addressed by IDT.
Establishes service provider linkages with allied health and social service agencies and institutions on behalf of hospice patients/families with appropriate follow-up and advocacy.
Maintains written records per hospice policies in a timely manner.
Maintains a high level of professional competence with ongoing development of social work practice and interdisciplinary issues related to death, dying, and bereavement.
Coordinates the delivery of social work services to maximize the effectiveness and eliminate duplication with contracted social service units of hospitals, County and State departments of Social Services, and other public and private agencies.
Maintains continuing education records and participates in workshops to keep updated on current social services practices/rules/regulations.
Maintains confidentiality of information.
Exhibits good listening and comprehension.
Uses negotiation skills to resolve conflicts.
Deals effectively with difficult situations.
Treats patients, families, and staff with respect and compassion.
No gossip, does not harm others in conversation.
Performs other responsibilities as assigned.
This is a PRN (as needed) weekend position.
