Registered Nurse

Provide direct nursing care to residents in accordance with established standards of practice and facility policies.Conduct comprehensive nursing assessments and monitor residents' conditions.Develop, implement, and evaluate individualized care plans.Administer medications, treatments, and other prescribed interventions safely and accurately.Monitor residents for changes in condition and promptly notify the physician, Nurse Practitioner, or other appropriate healthcare provider.Respond appropriately to medical emergencies and changes in residents' health status.Perform and document nursing procedures according to facility protocols.Maintain accurate and timely documentation in the resident's medical record/EHR.Participate in admissions, transfers, discharges, and ongoing resident assessments.Coordinate care with physicians, nurse practitioners, CNAs, rehabilitation staff, social services, dietary staff, and other members of the interdisciplinary team.Provide direction, supervision, and clinical guidance to LPNs, CNAs, and other nursing personnel as permitted by applicable regulations and facility policy.Assist with wound care, infection prevention, fall prevention, pain management, and other resident-care programs.Monitor residents' nutritional, hydration, skin integrity, and functional status.Communicate effectively with residents and their families regarding the resident's plan of care and condition.Follow infection-control procedures and participate in quality assurance and performance improvement activities.Ensure compliance with facility policies, professional standards, and applicable New York State and federal requirements.Maintain confidentiality and protect residents' rights and dignity at all times.Complete all required documentation, reports, incident reports, and other nursing records accurately and timely.Participate in staff meetings, in-service education, and continuing education as required.

Qualifications Current and valid New York State Registered Professional Nurse (RN) license.Current CPR/BLS certification, as required by the facility.Previous experience in a skilled nursing facility, nursing home, long-term care, rehabilitation, or geriatric setting is preferred.
