Home Health Telephone Triage Nurse

Suburban Home Health is a joint venture between several Suburban Health Organization hospital systems in Indiana.

This allows us to work as an extension of our partner hospital systems to service homebound patients in our local communities.

The Triage Nurse responds to patient requests telephonically during assigned on-call hours, providing professional nursing assessment to determine the urgency of patient needs.

On-call hours are Monday through Thursday, 4:30 PM-8:00 AM the following day.

Interviews the patient and/or family to identify triage priority based on available information Calls for crisis intervention if appropriate Communicates and acts as a liaison with patients, family and other health professionals to ensure the needs of the patient are adequately addressed Provides education to the patient and/or family in accordance with the Plan of Care Responds to patient calls within 15 minutes Accurately documents all on-call activities via the on-call communication log Contacts physicians if PRN orders are needed in order for the on-call nurse to perform a call-back visit Contacts the regional on-call nurse and provides report when call-back visits are warranted Consults the Administrator on-call when necessary Adheres to agency documentation requirements to provide timely, detailed, accurate documentation of care or services provided
