Hospice Social Worker BSW/MSW
HOSPICE SOICAL WORKER BSW Required/MSW Preferred Part-time or Full-Time (24-40 hours/week) POSITION SUMMARY: Responsible for providing the psychosocial and spiritual assessment, consultation and other social services to patients, families and/or caregivers being served by Cherokee Regional Hospice.
Supports patients, families, and caregivers in decision making as it pertains to life-limiting issues.
Coordinates and supervises the Hospice Bereavement Program.
Responsible for the social work function of Cherokee Regional Home Health by providing assessment and consultation to patients, families and/or caregiver being served by Cherokee Regional Home Health.
Coordinates the We Honor Veterans program.
ESSENTIAL FUNCTIONS: Responsible for the completion of the Psycho-Social Assessment for the patient and family/caregiver.
This will be completed in the first five days following the patient's election of the hospice benefit and is a part of the comprehensive assessment.
Completes the Spiritual Assessment of the patient and family/caregiver and may collaborate with the Hospice Chaplain.

Masters prepared Social Worker will serve in the supervisor role as an active advisor consulting with the bachelors prepared social worker on assessing the needs of patients and caregivers, developing and updating the social work portion of the plan of care, and delivering care to patients and families.
Masters Prepared Social Worker will supervise bachelors prepared social workers at a minimum of every thirty days.
Supervision may occur in person, over the phone, through electronic communication or any combination thereof.
Documentation will be kept in the employee file.
Electronic or paper documentation is acceptable.
Refers patients and families to community resources when appropriate Assists with hospice education needs Participates in workgroups, special projects, and QAPI initiatives Assists patients and caregivers in accessing community or other resources and understanding associated documentation Facilitates completing of Advanced Care Planning activities Volunteer Coordinator Assigns volunteer coordinator activities to qualified personnel Initiates volunteer program development, planning and evaluation Coordinates volunteer training program and assignment of volunteers to patient care or other duties as directed Coordinates volunteer activities throughout the year and annual recognition Keeps current list of volunteers with contact information, special interests, skills and abilities.
Updates volunteer files annually Conducts ongoing evaluation of the volunteer program and commits to provide easy to retain and recruit volunteers Supervises volunteer function in the home to evaluate effectiveness according to he hospice plan of care Reviews volunteer documentation Performs annual volunteer evaluations Responsible fo...
