What You'll Do

As a Physical Therapist, you will evaluate, develop, and implement individualized treatment plans designed to help residents improve mobility, restore function, and maximize independence.

Responsibilities include:


* Perform comprehensive physical therapy evaluations.


* Develop and implement individualized treatment plans based on resident needs and goals.


* Provide skilled therapeutic interventions to improve strength, balance, mobility, coordination, and overall function.


* Monitor resident progress and modify treatment plans as appropriate to maximize outcomes.


* Collaborate with physicians, nursing staff, therapy professionals, and the interdisciplinary care team to coordinate resident care.


* Educate residents and families on therapeutic exercises, mobility techniques, safety strategies, and fall prevention.


* Accurately document evaluations, treatments, progress notes, and discharge planning.


* Participate in care planning and interdisciplinary discussions as appropriate.


* Build positive relationships with residents while providing compassionate, patient-centered care.
