Westview Healthcare Center is seeking a compassionate, experienced, and dedicated Licensed Clinical Social Worker (LCSW) - Program Manager to support our residents and help coordinate our social services programs in a skilled nursing facility setting.

This role works closely with residents, families, nursing and medical staff, administration, and community agencies to ensure each resident's emotional, social, psychological, and medically related needs are addressed with dignity, compassion, and respect.

What You'll Do


* Assist with planning, developing, implementing, evaluating, and directing the facility's social service programs.


* Collaborate with administration, medical staff, nursing, and other departments to identify and address resident needs.


* Assist with developing and maintaining department policies and procedures.


* Help identify and address the medically related social and emotional needs of residents.


* Support residents in achieving their highest practicable level of independence, self-care, and well-being.


* Provide medically related social services to promote the highest practicable physical, mental, and psychosocial well-being of each resident.


* Maintain accurate social service documentation, progress notes, reports, and required records.


* Provide residents and families with information regarding Medicare, Medicaid, financial assistance programs, and available community resources.


* Provide consultation and support to facility staff and community agencies regarding resident needs.


* Assist residents and families with emotional concerns, anxiety, stress, adjustment to facility placement, illness, disability, helplessness, and end-of-life concerns.


* Evaluate social and family information and assist with developing appropriate social service interventions.


* Coordinate social services with nursing, therapy, activities, dietary, medical, and other departments.


* Assist with scheduling outpatient appointments and coordinating on-site ancillary services, including optometry, podiatry, dentistry, and psychiatric services.


* Participate in discharge planning and coordinate appropriate community resources, equipment, referrals, and agency services.


* Develop and maintain positive relationships with residents, families, staff, healthcare providers, community agencies, and social service organizations.


* Assist with orientation and in-service training related to the social, emotional, and medical needs of residents.
