Position Responsibilities

As a Physical Therapist, you will help residents improve their strength, mobility, function, and independence through individualized, evidence-based care.

Responsibilities include:


* Perform comprehensive physical therapy evaluations and develop individualized treatment plans.


* Provide skilled physical therapy services in accordance with the California State Practice Act.


* Monitor resident progress and modify treatment plans as clinically appropriate.


* Supervise Physical Therapist Assistants (PTAs) and collaborate with Restorative Nursing staff to promote continuity of care.


* Complete accurate and timely clinical documentation, including treatment notes, progress reports, and recertifications.


* Collaborate with nursing and the interdisciplinary care team to support comprehensive resident care.


* Participate in care planning meetings and utilization reviews as needed.


* Educate residents and families regarding mobility, safety, home exercise programs, and discharge planning.


* Assess and coordinate appropriate durable medical equipment (DME), including walkers, wheelchairs, braces, and other mobility equipment.


* Promote safe mobility and functional independence throughout the rehabilitation process.
