Physical Therapy (PRN)

Physical Therapist - Home Health PRN We are looking for a Physical Therapist to join our Home Health team! This position provides skilled physical therapy services to patients in their homes, helping them improve mobility, regain independence, and achieve their highest level of function.

What You'll Do Evaluate patients and develop individualized plans of care based on their needs and goals.

Provide skilled physical therapy services in the patient's home.

Help patients improve strength, mobility, balance, and overall functional independence.

Educate patients, families, and caregivers on exercises, safety, and home programs.

Collaborate with physicians, nurses, therapists, and other members of the care team.

Complete timely and accurate clinical documentation.

Monitor patient progress and update treatment plans as needed.

Ensure care meets Medicare, regulatory, and company standards.

What We're Looking For Current Physical Therapist (PT) license in the state of practice.

Graduate of an accredited Physical Therapy program.

Current CPR certification.

Valid driver's license, reliable transportation, and auto insurance.

Strong communication, organization, and time-management skills.

Home Health experience preferred, but not required.
