Position Summary

We are seeking a licensed Occupational Therapist (OT) to provide patient-centered therapy services focused on improving residents' independence and quality of life.

This role involves evaluation, treatment planning, interdisciplinary collaboration, and documentation compliance.
Key Responsibilities


* Evaluate residents and develop individualized treatment plans


* Provide skilled occupational therapy services per physician orders


* Document evaluations, treatments, and progress notes in compliance with regulations


* Collaborate with physicians, nursing, and interdisciplinary team members


* Educate residents, families, and caregivers on treatment plans and home programs


* Participate in care planning, discharge planning, and team meetings


* Conduct staff training and in-service education


* Maintain compliance with state practice acts and facility policies

Qualifications


* Degree in Occupational Therapy (Bachelor's required; Master's or Doctorate preferred)


* Active Occupational Therapist license in South Carolina (in good standing)


* Experience in skilled nursing or rehabilitation setting preferred


* Familiarity with PointClickCare and Casamba preferred


* Strong clinical, documentation, and communication skills
