SILP Case Manager

I.

JOB SUMMARY The SIL Case Managerprovides professional-level service coordination and case management for youths living on campus as part of our SIL program.

A case manager interacts with both individuals and families.

They are to provide emotional support to clients, while helping them structure their finances and plan their life goals along a structured program.

Compassion and empathy are a requirement of this position.

II.

DUTIES & ESSENTIAL JOB FUNCTIONS Develop strategies for sustaining and growing effective programs.

Build high-quality relationships with youth participants to understand personal experiences.

Provide strategic direction relating to ongoing program development, quality improvement, and evaluation efforts.

Implement core principles and evidence-based practices including, but not limited to: Positive Youth Development and Trauma Informed Care Complete developmentally appropriate assessment activities with youth as determined by program and youths needs.

Connect to immediate and long-term resources in the community Organize and maintain case notes, assessments, case summaries, case records, and other related records.Responsible for compiling, organizing, and preparing written reports and other written documents.Responsible for presenting case materials at internal planning meetings, court hearings, foster care reviews, and other assigned meetings.Other duties as assigned.

Work Environment/ Remote Office Requirements This position requires the ability to operate in a professional office environment.

This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Position Type/Expected Hours of Work: This is a full-time position.

The schedule requires flexibility based on the availability of the clients served.

Eligible for flex schedule.

Non-essential during inclement weather.

Travel: Travel is required and will include local as well as out-of-area travel.

IV.

BASIC COMPETENCIES Education, Experience and Skills Required: Bachelors degree in social work or related field.

1 - 3 years of experience working in human services with serious emotional disorder (SED) at-risk youth males or females between the ages of 16 19 preferred.

Maintain a valid Michigan drivers license and driving record that allows the operation of an agency vehicle.

One years experience working within community systems, referring, case management.

Proven ability to maintain high productivity while functioning in a flexible environment.
