OBAT Care Coordinator

OBAT Care Coordinator Are you looking for meaningful work where you can help people build healthier, more stable lives? Valley Community Services Board is seeking a Care Coordinator to join our Office Based Addiction Treatment (OBAT) team.

In this role, you'll work directly with individuals experiencing opioid and/or alcohol use disorders, helping them access treatment, overcome barriers to care, and connect with the resources and support they need for recovery.

You'll be part of a collaborative treatment team serving individuals and families throughout Staunton, Waynesboro, Augusta County, and Highland County.

This position combines office-based services with work in the community, giving you the opportunity to build relationships and make a direct impact on the people we serve.

Responsibilities include: Provide ongoing substance use care coordination for individuals with opioid and/or alcohol use disorders.

Meet with individuals in office, community, and home-based settings throughout our service area.

Assess each individual's strengths, needs, goals, and barriers to recovery and develop individualized treatment plans.

Connect individuals with treatment providers, community resources, and other services that support recovery and stability.

Follow up on referrals, monitor progress, help solve barriers to care, and advocate for individuals as they navigate treatment.

Assist with substance use assessments for individuals referred to the OBAT program.

Coordinate with community agencies and treatment providers, including referrals to inpatient or residential treatment when a higher level of care is needed.

Work closely with the OBAT team through weekly staff meetings and ongoing clinical and administrative supervision.

Maintain timely, accurate documentation in accordance with Medicaid and program requirements.

Complete monthly chart audits to support accurate care coordination billing.
