The Social Services Director is responsible for assessing residents' psychosocial needs and coordinating services that promote the highest practicable physical, mental, and psychosocial well-being of each resident.
This role works closely with residents, families, and the interdisciplinary team to support quality of life, discharge planning, and resident advocacy while fostering a compassionate, resident-centered environment.
Skills Needed:
Resident Advocacy & Psychosocial Support: Assess residents' psychosocial needs, develop appropriate interventions, and provide emotional support to residents and families throughout their stay.
Care Planning & Collaboration: Participate in interdisciplinary care planning, review resident needs, and collaborate with nursing and other departments to ensure individualized, person-centered care.
Communication & Conflict Resolution: Effectively communicate with residents, families, ombudsmen, and the care team.
Mediate resident and family concerns, including roommate conflicts, room changes, and other sensitive situations with professionalism and compassion.
Discharge Planning: Coordinate discharge planning, identify potential social or economic barriers to a successful discharge, and connect residents and families with appropriate community resources and referrals.
Organization & Documentation: Maintain accurate documentation, ensure compliance with state and federal regulations, and manage multiple priorities in a fast-paced long-term care environment.
Requirements:
* Bachelor's degree in Social Work, Human Services, or a related field preferred.
* Experience in a long-term care, skilled nursing, rehabilitation, or healthcare setting preferred.
* Experience working with geriatric residents, including those with dementia or behavioral health needs, preferred.
* Strong communication, conflict resolution, organizational, and interpersonal skills.
* Knowledge of discharge planning, community resources, and resident rights preferred.
