Case Manager Registered Nurse

Position Summary A clinically competent Registered Nurse who is responsible for assisting in carrying out the hospitals Case Management Plan.

Educates staff regarding effective allocation of the Hospitals resources while encouraging provision of high-quality patient care.

Is cognizant of regulations and policies of all review organizations relating to billing of the fiscal intermediarys coordinates, monitors and evaluates options in collaboration, and communication with other staff members.

Assesses, plans, implements, coordinates, monitors, and evaluates options to facilitate the continuum of care.

POSITION SUMMARY EXPANDED: Under the Oklahoma Medical Marijuana and Patient Protection Act, (The Unity Bill) this position is considered a Safety-sensitive position.

Safety-sensitive is defined to include any job that includes tasks or duties that could affect the safety and health of the employee performing the task or others.

Employees working in safety-sensitive roles are subject to this exception and subject to disciplinary action in the event of a positive test for marijuana or its metabolites.

EDUCATION REQUIREMENTS Graduate of an accredited school of nursing and possess current license in the State of Oklahoma.

Minimum of three (3) years acute hospital nursing or swing bed experience preferred and three (3) to five (5) years acute hospital or swing bed case management experience preferred.

Knowledge of theory, principles, techniques, and practice of professional nursing; of scientific principles and nursing research; of preparation, administration, and action of medications; of treatments and medical procedures; of counseling and teaching techniques; of medical equipment as it relates to patient care.

Emergency Room experience preferred.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED Licensed RN in the state of Oklahoma.

BLS certification required upon hire.

PALS preferred but not required.

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES) Knowledge of the Continuum of Care and Utilization Processes Knowledge of state and federal programs that provide medical care and financial support to individuals and community resources.

Ability to demonstrate clinical competence and maintain current knowledge in caring for pediatric through geriatric patients.

Ability to document care management plans in a clear and concise manner.

Ability to communicate effectively both verbally and in writing.

Ability to work with diverse patient and staff populations with knowledge and sensitivity to cultural and spiritual influences impacting on patient care.

Ability to work independently, with no direction.
